6 Quick Apply 17 hours ago Full Job Description POSITION SUMMARY The Licensed Practical Nurse (LPN) functions as a team member in the home health setting under the general supervision of the registered nurse and in accordance with patients plans of care.
The LPN can perform nursing care, record the nursing care given, and report to the appropriate person changes in patient condition that are within the practice scope of their LPN license.
The LPN is assigned to work a rotating weekend every 4 weeks and provide coverage for one summer and one winter holiday, plus the associated holiday weekend if applicable.
The LPN does have an option to choose every other weekend rotation if requested.
TECHNICAL RESPONSIBILITIES Performs nursing interventions and provides personal care to patients with basic and complex health needs under the general direction of a registered nurse and according to a plan of care which has been created by the registered nurse.
Applies preventive nursing measures in providing care to patients.
Demonstrates ability to collect information and observe signs and symptoms which indicate changes in physical and psychosocial condition of the patient.
Assists patients to increase independence and self-reliance as their condition allows.
Performs specialized treatments in RN-directed nursing care situations.
Knows and can initiate interventions and perform effectively in emergency situations.
Is knowledgeable and adept in the use of effective communication skills with patients, families, and other team members.
Demonstrates recognition and respect for the values of self and others; able to recognize differences between values of self and others and administers care impartially regardless of differences.
Contributes to the development and revisions of basic patient care plans.
Pursues new learning opportunities to ensure capability with all patients.
JOB REQUIREMENTS Licenses:
Wisconsin Licensed Practical Nurse (LPN) Licensure.
Education:
High school diploma or equivalent required.
Experience:
One-year experience as a LPN with a Medicare-certified, skilled intermittent home health agency preferred.
Experience with computerized home health documentation system preferred.
Miscellaneous:
CPR certification required.
Valid Wisconsin driver's license and current auto insurance required.
Must possess knowledge of general patient care and ability to perform nursing procedures, knowledge of the psychosocial needs of patients and families, knowledge of patient behavior and symptoms for a wide range of injuries and illnesses.
Must have the ability to make accurate patient observations, communicate and record them, and provide guidance for patients in participation of their care plan.
Knowledge of the effects of medication and ability to observe and report them required.
Must be able to perform effectively in emergency situations.
Ability to function as a team member and to contribute to overall planning for patient care and ability to communicate effectively with patients, families, and team members required.
Must be able to prioritize, organize and efficiently complete work and be a leader within the department.
